HOW DO I...
REGISTER WITH THE PRACTICE?

To register as a patient within the practice, please drop into the health centre and ask our reception staff for a new patient registration pack. Complete the necessary forms and return them to the practice. We may ask that you attend for a registration examination if you have any past or current medical conditions. We will carry out some basic tests such as weight, urinanalysis, possibly some blood tests and a medication review. This information is invaluable as it allows us to start any treatment immediately. If you have your medical card it would be useful to bring this along.
